Job Description: Licensed Practical Nurse (LPN) / Registered Nurse (RN)

Position Summary
The LPN/RN at Young 60 Plus Club plays a vital role in ensuring the health, safety, and overall well-being of adult day service participants. This position involves providing direct nursing care, conducting health assessments, administering medications, coordinating care with external providers, and supporting staff education. Qualifications Licensure & Certifications
- Must hold a current, valid LPN or RN license in the State of Illinois.
- Certification from a licensed physician within 30 days of employment confirming good health and freedom from communicable diseases (including tuberculosis) per IDHS requirements.
- Current CPR/BLS certification; other credentials as required by state regulations.
- Must pass all required background checks and screenings.
Education & Experience
- Graduate of an accredited school of nursing.
- Minimum of one year of clinical experience in geriatrics, adult day services, long-term care, or a related healthcare setting preferred.
- Experience working with adults with dementia, Alzheimer’s disease, or cognitive impairments is strongly preferred.

Key Responsibilities
- Conduct Health Assessments: Perform initial and ongoing nursing assessments for all participants.
- Develop Care Plans: Create, implement, and regularly update individualized care plans in collaboration with families, physicians, and the care team.
- Monitor & Document: Observe participant health, document changes, and promptly report significant findings to physicians, caregivers, and administration.
- Administer Care: Provide medications, treatments, wound care, and health screenings while maintaining accurate medical records.
- Coordinate External Care: Liaise with primary care providers, specialists, home health, and therapy services to ensure continuity of care.
- Support Infection Control: Implement infection prevention measures, monitor communicable diseases, and train staff on proper protocols.
- Emergency Response: Act promptly in emergencies, administering first aid or interventions as necessary.
- Health Education: Provide education and counseling to participants and families on wellness, safety, and disease management.
- Staff Support & Training: Serve as a clinical resource; assist with staff training on participant care and safety practices.
- Quality Assurance: Participate in audits, compliance checks, and quality improvement initiatives.
- Regulatory Compliance: Ensure adherence to IDHS, state, and federal guidelines governing adult day service nursing care.
- Professional Development: Maintain active licensure and complete continuing education as required.
- Deliver personalized patient care by assessing health status, administering medications, and monitoring vital signs during daily visits.
- Maintain accurate and detailed medical documentation, including progress notes, treatment plans, and incident reports, adhering to HIPAA guidelines for patient privacy.
- Collaborate with multidisciplinary teams to develop and implement individualized care plans tailored to each participant’s needs.
- Ensure all medical records are up-to-date and compliant with regulatory standards, including proper documentation of treatments and interventions.
- Educate patients and their families about health management strategies, medication usage, and wellness practices.
- Uphold safety protocols by conducting routine checks of the facility environment and responding promptly to any medical emergencies or changes in patient condition.
- Maintain thorough knowledge of medical terminology, physiology, and anatomy to support accurate assessments and effective care delivery.
- Other Duties: Perform additional nursing tasks as assigned by the Administrator/COO or President & CEO to support the organization’s mission.
Work Environment & Physical Requirements
- Work performed in an adult day services center with daily interaction with older adults, families, and interdisciplinary team members.
- Must be able to lift, push, pull, or move up to 25 pounds.
- Must respond calmly and efficiently in emergencies.
