Summary
We are seeking an experienced and mission-driven Case Manager Supervisor to lead our case management team across all housing programs. This individual will oversee the development of service plans, monitor the efficiency of service delivery, and ensure participants are connected to the resources they need to achieve self-sufficiency and housing stability.
Responsibilities
Supervisory & Administrative Duties
- Lead, supervise, and support a team of 3–4 case managers
Case Management Oversight
- Coordinate housing referrals, interviews, and admissions
- Support the development and review of individualized service plans
- Ensure clients are connected to appropriate services (mental health, employment, benefits, etc.)
- Track client progress toward autonomy, wellness, and housing stability
- Assist with crisis intervention and manage high-need cases
- Participate in team meetings, community collaborations, and resource-building efforts
Client & Community Engagement
- Help clients with apartment searches and securing basic needs
- Connect participants with external programs and supportive services
- Engage in planning events and activities that enhance quality of life
- Follow up with discharged clients to evaluate long-term outcomes
- Assign caseloads, set schedules, evaluate staff performance, and support professional development
- Oversee case documentation in HMIS and Apricot systems
- Coordinate reporting, compliance, and annual recertification processes
- Identify and implement department improvements and best practices
Requirements
Education & Experience
- Bachelor’s degree in Social Work, Human Services, or a related field (required)
Preferred Skills
- Strong verbal and written communication
- Crisis management and advocacy skills
- Effective multitasking and organizational abilities
- Ability to work both independently and collaboratively
- Bilingual (Spanish/English) a plus
- Valid driver’s license preferred
- 3 years of experience working with vulnerable populations, preferably women and families
- 1–2 years of supervisory or team leadership experience preferred
- Familiarity with HMIS, Apricot, Microsoft Office, and Google Suite
