The CHAP/CHEER Case Manager at the Greater Waterbury YMCA provides case management for clients in the CHAP/CHEER program.
Qualifications
- Bachelor’s Degree in closely related field (Social Work, Secondary Education or Counseling)
- Experience in teaching and reinforcing life skills and assistance with community linkages for development of natural supports
- The ability to work professionally with others, as well as autonomously
- Minimum of two years of experience working with youth.
- Excellent interpersonal communication and writing skills, well-organized, the ability to manage your time effectively, and operate business computer applications at an intermediate level (Microsoft Word, Excel, Business Email Systems, etc.)
- Must have and maintain a clean CT Driver’s License, current state registration and be the policy holder of full auto insurance coverage
- Provide 5-40 hours of case management services
- Must have safe, reliable vehicle. Mileage reimbursement paid monthly.
- Teach and reinforce all life skills documented on the Life Skills Inventory checklist, including budgeting, housing, education and employment
- Be knowledgeable of community resources and provide information and assistance with community linkages for development of natural supports
- Assist clients with finding and enrolling in suitable secondary education and obtaining financial aid
- Monitor educational status including attendance and grades
- Assist clients with preparing a resume and job searches
- Assist new clients and those wishing to move with locating and maintaining housing
- Maintain knowledge about program guidelines and DCF policies and procedures
- Ensure required documentation and reports are completed and presented as required
