Essential Responsibilities
Program Leadership & Strategy
- Design and execute a comprehensive obesity awareness strategy aligned to grant goals.
- Stand up and manage community hub to deliver education, screenings/referrals, and navigation.
- Develop annual/quarterly work plans and sustainability strategies.
- Develop/adapt culturally responsive training curricula on obesity awareness and healthy behaviors.
- Facilitate train-the-trainer sessions for coordinators and partner staff.
- Implement quality assurance and continuous improvement cycles.
- Recruit, hire, onboard, and performance-manage part-time coordinators and volunteers.
- Provide coaching, weekly check-ins, and field support.
- Manage timesheets, mileage, and compliance documentation.
- Create community engagement committee
- Establish MOUs and operating rhythms with CBOs, faith-based partners, and healthcare systems.
- Build referral relationships with providers across Delaware.
- Coordinate joint events, co-branding, and data protocols as permitted.
- Oversee hub schedules, outreach calendars, supplies, and safety protocols.
- Manage the program budget, reconcile spend, and forecast needs.
- Ensure compliance with all grant requirements.
- Implement standardized data collection across hub.
- Maintain accurate, timely records in approved systems.
- Produce monthly dashboards and narrative reports for funders.
- Develop outreach materials and messaging tailored to Delaware communities.
- Represent the organization at coalitions, health fairs, and partner meetings.
- Share success stories to build trust and visibility.
- Strong interpersonal and communication skills to serve a diverse community.
- High school diploma or equivalent
- Two years of progressive work experience in a health and wellness environment
- Strong data literacy and grant reporting skills.
- Valid driver’s license: statewide travel and some evening/weekend work required
- Multi-lingual
- 3 years leading community health or chronic disease prevention programs.
- Proven success building partnerships with CBOs, faith-based organizations, and healthcare providers
- Excellent facilitation and training skills
- Experience with obesity, nutrition, behavioral change, and SDOH programming.
- Familiarity with HIPAA, referral platforms, and closed-loop referral systems.
- Prior grant management and budget oversight experience
Salary : $50,000 - $60,000
