For this New York United States-based position, the expected base pay is $30.00 to $34.00/HR based on experience
Below is a list of your responsibilities as an Discharge Planner with YesCare:
- Develop and implement comprehensive discharge plans, including mental health, SUD, and community-based follow-up care to support successful reintegration.
- Assess patient needs and identify appropriate community resources based on clinical level-of-care requirements.
- Complete all required transfer and discharge documentation, including when individuals transition to state prison upon sentencing.
- Document discharge planning activities in the medical record and maintain an accurate, up-to-date discharge planning log.
- Serve as an active member of the multidisciplinary case management team, coordinating care with physicians, mental health providers, therapists, probation/parole, care managers, and community partners.
- Complete Single Point of Access (SPOA) applications to connect clients with mental health, behavioral health, and developmental disability services.
- Connect patients with severe mental illness to the Transition Manager Program at Access Supports for Living prior to their release.
- Collect and report monthly Quality Improvement (QI) data, including pregnancy tracking, chronic care lists, mental health indicators, and SUD/MAT-related metrics.
- Provide administrative support, including answering calls, documenting messages, preparing correspondence for mandatory court-ordered HIV/STD testing, and assisting with patient communication needs.
- Maintain appointment rosters and schedules for on-site and off-site services to ensure proper coordination between internal staff and external providers.
- Maintain organized and accurate medical records filing, ensuring documents are properly categorized and accessible for clinical use.
- Provide crisis support and de-escalation when needed, assisting with stabilization and linkage to appropriate follow-up services.
- Perform other duties as needed to support continuity of care, clinical operations, and reentry coordination.
- Bachelor’s degree in social work, psychology or related field.
- Experience in mental health, housing stabilization, community reintegration, SUD services, and care management highly preferred.
- Skilled in crisis intervention, motivational interviewing, MAT coordination, and linkage to community supports ideal.
- CPR/AED Certified, Narcan Certified, MAT Training preferred.
- Knowledge of community mental health resources, Medicaid/benefits navigation, and care coordination.
