We are seeking a warm, reliable, and motivated Spanish-speaking Care Manager with Health Homes experience to support families in Queens, NY. This position begins with a light caseload, then gradually increases as you become a trusted representative of our company.
About the Role
As a Care Manager, you will:
- Support Spanish-speaking children and families enrolled in the Health Homes Serving Children program
- Complete required core services, assessments, and member contacts
- Conduct in-person and telehealth visits based on acuity level
- Build meaningful relationships with families, schools, and providers
- Advocate for services, assist with paperwork, and ensure families feel supported
- Travel within Queens for scheduled visits
- Represent our company with professionalism, warmth, and respect
What We’re Looking For
- Health Homes experience (required)
- Fluent in Spanish and English
- Passion for helping children and families
- Strong communication and organizational skills
- Ability to travel within Queens
- Experience with CANS-NY, care coordination, or case management is a plus
