AREA/DEPT: Yates Safety

SHIFT: Day Shift

PAY: Salary Position – Pay will be based initially on experience.

RESPONSIBILITIES:

· Assessing and analyzing an injured employee to evaluate the medical and vocational needs required to facilitate the patient’s appropriate and timely return to work. Acts as a liaison with patient/family, employer, provider(s), insurance companies, and healthcare personnel.

· Travel to and meet with claimants, physicians to determine status, treatment plan, progress, accommodations for return to work.

· Assesses and analyzes an injured employee’s medical status; develops a plan of care to facilitate the patients’ appropriate medical care and timely return to work.

· Interviews in person or over the phone, or physician’s office to provide ongoing case management services.

· Communicates both in-person and via telephone with patients, medical providers, attorneys, employers, and insurance carriers; prepares all required documentation of case work activities.

· Arranges referrals, consultations, and therapeutic services for patients on an as needed basis; confers with specialists concerning courses of care and treatment.

· Adheres to all laws and regulations that apply to the provision of rehabilitation services applies all special instructions required by individual insurance carriers and referral sources.

· Testifies as required to substantiate any relevant case work or reports.

· Performs other related duties as assigned.

JOB REQUIREMENTS

· Background in Workers' compensation, neurosurgery, orthopedics, occupational health, pain management, utilization review, and home health is a plus.

· The level of education desired for candidates in this position is an R/N or LPN or equivalent experience.

· Nursing / Registered Nurse/LPN is required.

· Certified Case Manager (CCM) or Certified Occupational Health Nurse (COHN)

· Required travel up to 25%

Licenses and Certifications:

· Nursing / Registered Nurse/LPN is required.

· First Aid / CPR / AED is required.

RN/LP and one of the following certifications is required:

· Certified Case Manager (CCM) or Certified Occupational Health Nurse (COHN)

Functional Experience:

· Medical Management / Medical Management - Case Management / 3 years

· Nursing / Occupational / 3 Years

· Nursing / Workers' Comp / 3 Years
