What are the responsibilities and job description for the Service Coordinator - Developmental Disabilities position at Yamhill County?
Yamhill County has one regular full-time position for a Services Coordinator in our Intellectual and Developmental Disability Program. In this role you will be responsible for providing case management to children and adults with developmental disabilities. Your job duties will include case management, needs assessments and documentation, developing and monitoring individualized support plans (ISPs), team facilitation and problem-solving, providing consumers with information about available services, and other responsibilities as assigned.

The Qualifications:
Required
- Bachelor’s degree in social work, psychology, counseling, or education.
- Two years’ experience in a human services field
- Preferably with the Developmentally Disabled population.
OR
- A high school diploma.
- Five years’ work/relevant experience in human services.
Any satisfactory equivalent combination of experience and training which ensures the ability to perform the work may substitute for the above.
Preferred
- Basic knowledge of the public system for Developmental Disabilities services in Oregon
- Fluency in both English and Spanish
A valid license to drive in Oregon, proof of auto insurance, and an acceptable driving record are required. You must pass a criminal history background check.
