About the Role
As an MDS Coordinator, you will be responsible for developing comprehensive care plans and ensuring that the care provided meets the needs of our residents.
Responsibilities
- Keep abreast of current federal and state regulations, as well as professional standards.
- Assist with the development of comprehensive care plans for Residents in coordination with the MDS that accurately addresses the needs of the Resident.
- Coordinate, manage, and monitor the written plan of care for each Resident of the facility that identifies the needs of the Resident and goals to be accomplished for each need.
- Assist nursing management with the coordination, management, and review of nurse’s notes to determine if the care plan is being followed.
- Monitor Resident status changes to ensure appropriate and timely nursing or clinical team involvement.
- Ensure MDS and support documentation are accurate representations of the Resident and meet regulatory and auditor requirements.
- Perform regular audits of documentation to assure accuracy.
- Assist nursing management with the discharge process.
- Perform administrative requirements, such as completing necessary forms and reports.
- Ensure that established infection control and standard precaution practices are maintained when providing care.
- Maintain strict confidentiality regarding sensitive health information of Residents.
- Report all hazardous conditions, damaged equipment and supply issues to appropriate persons.
- Maintain the comfort, privacy, and dignity of Residents and interact with them in a manner that displays warmth, respect, and promotes a caring environment.
- Answer and respond to call lights promptly and courteously when working in Resident care areas.
- Communicate and interact effectively and tactfully with Residents, visitors, families, peers, and supervisors.
- Attend and participate in departmental meetings and in-services as directed.
Qualifications
- Current State License as a Nurse
- C.P.R. Certified
- MDS experience
Required Skills
- Knowledge and training in all aspects of MDS process.
- One year experience as a MDS Nurse in a long-term care setting.
