Pay rate: $35 plus .70 cents per mile reimbursement

About Us:
Complete Home Care helps Indiana Medicaid members stay safe and independent at home through the Attendant Care and Structured Family Caregiving (SFC) programs. We're looking for a friendly, organized Home Health Coach who enjoys getting out into the community and building relationships with clients and families.
What You'll Do:
- Conduct in-home visits with clients monthly (Attendant Care) and quarterly (Structured Family Caregiving)
- Check on client well-being, service satisfaction, and any changes in needs
- Support and coach caregivers, answering questions and reinforcing care plans
- Complete visit documentation accurately and on time
- Coordinate and attend joint visits with Medicaid case managers as needed
- Report concerns, incidents, or changes in condition to the office promptly
- Serve as a warm, professional point of contact for clients and their families
What We're Looking For:
- Experience in home care, home health, or Medicaid waiver services
- Comfortable driving to and conducting visits in clients' homes
- Excellent customer service and communication skills
- Organized and dependable with paperwork and deadlines
- Able to work independently and manage a caseload
- Valid driver's license, reliable transportation, and auto insurance
- Must pass background check and meet agency health screening requirements
Pay: $35.00 per hour
