Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to inform care plans.
- Administer medications and injections accurately following physician orders, including IV infusions and botulinum toxin treatments.
- Assist with medical procedures such as catheterization, suturing, tube feeding, and specimen collection/processing.
- Monitor patients’ responses to treatments like ventilator management, pain management protocols, and infection control measures.
- Document all patient interactions meticulously using EMR (Electronic Medical Records) systems such as Epic or Cerner; ensure compliance with HIPAA regulations.
- Collaborate with multidisciplinary teams on discharge planning, case management, and post-acute care strategies to optimize patient outcomes.
- Provide specialized care for individuals with dementia or Alzheimer's disease in memory or hospice care settings; support patients with developmental disabilities when needed.
Requirements
- Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice.
- Proven experience in diverse clinical environments such as hospitals (including ICU or Level I Trauma Centers), nursing homes, or outpatient clinics.
- Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), and medication administration protocols.
- Experience with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks is highly preferred.
- Demonstrated ability to perform complex procedures including IV insertion, phlebotomy, sterile techniques, and airway management.
- Critical care experience such as ICU or emergency room exposure is desirable; familiarity with acute pain management and infectious disease care is a plus.
- Excellent patient assessment skills with the ability to work effectively with seniors, children—including neonatal ventilator care—and individuals with disabilities.
- Knowledge of HIPAA compliance standards and infection control practices essential for maintaining patient confidentiality and safety.
