What are the responsibilities and job description for the Behavioral Care Manager position at Wyckoff Heights Medical Center?
Provide and coordinate behavioral health care management for patients receiving HIV prevention and care services in the Wyckoff Medical Arts building, an outpatient clinical site of that provides primary care, HIV medical care, and supportive services.
- Demonstrate effective communication skills necessary to interpret age appropriate response to service/treatment plan.
- Complete in a timely fashion other job-related duties to meet program needs as assigned by the Program Director.
- Attend necessary training and in-service.
- Participates in regular supervision with immediate supervisor and team meetings with other clinic and program staff as required.
- Utilize program-identified evidence based interventions and other service delivery best practices to implement appropriate client-centered counseling and education.
- Maintain an electronic registry of all patients participating in the program, and ensure complete and timely documentation of services delivered in the EMR.
- Facilitate teaching and consultation to other health professionals on behavioral health topics, appropriate screenings and interventions, and the referral process.
- Communicate regularly and participate in case conferences with clients’ assigned primary care provider, linkage specialist/navigator, and other support staff to discuss recommendations and ensure a team-based approach to the client’s care plan.
- Participate in regular supervision with the clinical supervisor to review individual cases, treatment plans, and discuss recommendations to engage difficult cases.
- Monitor patient progress and outcomes closely using standardized measures.
- Consult on an ongoing basis with patients and primary care providers about treatment options and preferences.
- Conduct individual counseling and psychotherapy services to address the most pressing behavioral health needs of the client.
- Provide psychoeducation to clients on treatment options for behavioral health issues and design a client-centered treatment plan.
- Conduct mental health evaluations of clients who are referred for psychotherapy after receiving universal mental health and substance use screenings in the clinic.
- Perform other duties as assigned.
Position Requirements:
* This position requires a minimum formal education of Master Degree and a minimum of 5 years job-related experience.
* Desired (not required) criteria include: Minimum certification of a NYS-approved mental health license, including LMSW or LMHC. 1 years’ experience providing behavioral health services to underserved communities.
* License NYS- Approved Mental Health, including LMSW or LMHC.
* Experience providing mental health services to clients who receive HIV services and/or to special populations. Intermediate computer experience, but is not limited to proficiency in Microsoft Office Suite. Fluency in Spanish highly preferred.
