Responsibilities also include consulting with physicians and the health care team regarding patient care, participating in team and patient care, participating in team and patient/family conferences, educating physicians and associated to speech-language interventions to insure continuity of care, and completing all required paper work such as clinical records, care plans, and charge sheets.

Minimum Qualifications
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
- Master’s Degree from a school of Speech Language pathology.
- Current West Virginia SLP Therapy Licensure, or applicable state licensure where services will be performed.
- CPR certification within 30 days of hire date.
- Valid driver’s license.
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
- Member of (ASLHA) American Speech Language Hearing Association.
- Must possess valid WV driver’s license and must have automobiles insurance of $1000,0000/3000,0000 on his/her vehicle.
- Performs evaluation procedures according to policy/procedures according to policy/procedure or standard of care including but not limited to: dysphagia evaluation, voice evaluation, communication evaluation, cognitive assessment, language evaluation, hearing screens.
- Safely and effectively delivers and analyzes speech-language interventions as per clinical policies and procedures.
- Implements home programs, meets patient/family educational needs as per policy and procedure.
- Promptly and thoroughly completes required clinical documentation including evaluations, progress reports, discharge summaries, and care plans.
- Instructs and delegates restorative speech language programs to the nursing staff to insure continuum of care as per policy and procedure.
- Calls patients timely and follows visit pattern.
- Performs CPR when directed by patient’s/appropriate representative’s wishes.
- Demonstrates bag technique per policy and procedure.
- Apply and maintain equipment according to the Policy and Procedure Manual.
- Follows supply requisition systems as per policy and procedure.
- Evaluates the effectiveness of the plan on a regular basis.
- Coordinates the care team in order to facilitate a change in the level of care in compliance with the change in level of care standard.
- Identifies the stages of death and dying
- Frequent walking, standing, and sitting.
- Must be able to position patients for effective delivery of care.
- Must be able to physically demonstrate Speech – Language interventions.
- Must be able to perform the physical task of written communication.
- Must have adequate visual and hearing acuity to allow for competent delivery of care.
- Exposure to bodily fluids, human tissues, contagious diseases, sharps, and explosive gases.
- Exposure to toxic gases, fumes, and odors.
- Exposure to high stress and constant interruptions.
- Exposure to electrical current.
- Exposure to radiation from x-rays.
- Exposure to bad road conditions.
- Exposure to wind, rain, snow, sleet, mud, high and low temperatures, animals and insects, and hazardous roadways.
- The majority of work is performed in the patient’s home and may require extensive travel.
- Subject to unpredictable situations. May be exposed to adverse or unpleasant home environments.
- Must have strong communication skills, written and verbal.
- Must demonstrate good initiative and sound judgment in the evaluation and delivery of speech-language services.
- Must be able to perform the physical task of written communication.
- Must have good time management skills.
- Demonstrates knowledge of and compliance with safety policies and procedures.
