SALARY: $27 -$29 per hour

Lead Licensed Practical Nurse (Lead LPN)**

Your Key Responsibilities:
Leadership & Oversight

  • Create and manage work schedules for LPNs within the unit, ensuring adequate coverage and continuity of care.
  • Review and approve LPN timesheets and leave requests in accordance with facility policies.
  • Monitor daily workflow of the unit to ensure efficiency, quality care, and timely task completion.
  • Assign duties and responsibilities to LPNs based on resident needs, staff competencies, and operational priorities.
  • Review work products of LPNs, providing feedback and guidance to maintain standards of care and documentation.
  • Identify operational challenges within the unit and develop practical solutions to improve workflow, and communication.

Patient Care

  • Oversee and support LPNs in delivering comprehensive shift reports to ensure seamless transitions and continuity of care.
  • Ensure proper medication counts and documentation are completed accurately and safely.
  • Supervise LPNs assisting residents with self-medication programs, promoting independence and health literacy.
  • Provide guidance on basic wound care procedures and ensure adherence to clinical protocols.

Documentation

  • Ensure timely and accurate documentation of changes in residents' status or behavior by LPN staff.
  • Monitor the quality of vital sign recordings and fingerstick blood sugar (FBS) documentation.
  • Audit clinical records for completeness and compliance with regulatory standards.

Emergency Preparedness

  • Ensure LPN staff are trained and participate in regular emergency drills to maintain readiness.

Medication Knowledge

  • Provide mentorship to LPNs on medication administration, dosage calculations, and side effect monitoring.
  • Ensure residents receive appropriate education on medication usage and health management strategies.

Teamwork & Development

  • Foster a supportive and cohesive team environment among LPNs.
  • Lead problem-solving and decision-making efforts to enhance care quality and operational efficiency.
  • Provide coaching and development opportunities for LPN staff to advance clinical skills and professional growth.

Current License to practice as a Licensed Practical Nurse in the state of West Virginia.

Experience
: 2 years of clinical experience as a Licensed Practical Nurse (LPN)
