We are excited to announce a Full-Time RN Case Manager position for a wonderful home health client in Longmont. This role offers a mix of in-person patient visits (60%) and remote work (40%).
What You'll Do
- Manage a patient caseload and ensure high-quality, timely care
- Oversee and support Home Health Aides/HHAs
- Complete patient assessments and develop personalized care plans
- Coordinate care with primary care providers, therapy teams, and our internal staff
- Educate patients and families on their care needs
- Maintain accurate electronic documentation with a quick 24-hour turnaround
- Participate in on-call rotation (about one weekend/month), holiday rotation, and occasional weekday phone triage
- Hybrid role involving 60% in-person and 40% remote work
- Local travel may be required
- $80,000-$84,000 annual salary with bonus potential
