Responsibilities:
Continuously assesses residents' needs and strengths to provide appropriate support and establish achievable goals.
Helps residents identify, respond to, and cope with life stressors.
Hosts support meetings and collaborates with external vendors to organize group classes as needed.
Maintains communication records for all residents and service notes.
Schedules annual check-in meetings for individuals and guardians.
Sends out client satisfaction surveys and documents results.
Works as a team with other social workers to maintain a comprehensive understanding of the organization and provide coverage for each other.
Collaborate with interdisciplinary teams to ensure coordinated and effective care.
Maintain accurate and confidential client records in compliance with all regulations and ethical standards.
Advocate for clients' rights and access to resources within the community.
Participate in case conferences, staff meetings, and professional development activities.
Qualifications:
Master's degree in Social Work (MSW) from an accredited institution.
Current and valid Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) license in the state.
Experience in providing clinical social work services to individuals with Developmental Disabilities.
Strong communication, interpersonal, and organizational skills.
Ability to work independently and as part of a team.
Knowledge of community resources and social service systems
Valid driver's license and reliable transportation.
Successful completion of Background/Fingerprint check and Drug testing.
