What are the responsibilities and job description for the Regional Lead Care Manager - Des Moines position at Woven Home Care?
Lead a Market. Build a Team. Deliver Care That Matters.
Description
Woven Home Care is on a mission to make home the safest, most effective place to receive care. We leverage our proprietary technology to enhance the work of our team and the care of our clients. As we expand into new markets, we are seeking a Regional Lead to build and lead one of our newest regions - Des Moines, Iowa.
The Regional Lead is the accountable owner of the region’s performance — responsible for care quality, operations, team culture, referral growth, and financial outcomes. In early months, you will personally conduct client assessments and oversee care delivery. As the market grows, you will scale the team and transition into broader leadership.
If you are entrepreneurial, clinically grounded, operationally disciplined, and motivated to build something meaningful — this role was designed for you.
What You’ll Do
1. Deliver Exceptional Client Care
- Conduct comprehensive in-home client assessments.
- Develop individualized care plans.
- Supervise and coach caregivers (“Weavers” aka our Home Care Aides).
- Ensure documentation, compliance, and safety standards are met.
- Conduct proactive client satisfaction check-ins.
- Address and resolve service concerns quickly and thoughtfully.
You will set the standard for quality from day one.
2. Build & Lead a High-Performing Care Team
- Interview, hire, and onboard caregivers.
- Provide mentorship, field coaching, and performance feedback.
- Create a caregiver-first culture grounded in professionalism and respect.
- Reinforce Woven’s high training and reliability standards.
- Partner with scheduling staff to ensure dependable coverage.
You are the foundation upon which great care is built.
3. Grow the Market Through Relationships
- Build referral relationships with hospitals, rehab centers, physicians, senior living communities, and elder care professionals.
- Conduct in-services and educational events.
- Attend community events and represent Woven publicly.
- Respond rapidly to referrals and convert assessments into long-term clients.
- Maintain disciplined CRM tracking and outreach cadence.
You will be the face of Woven in your region.
4. Own Operational & Financial Performance
- Oversee scheduling, documentation review, payroll inputs, and invoicing accuracy.
- Monitor KPIs including revenue growth, caregiver retention, client satisfaction, and utilization.
- Partner closely with corporate leadership through structured weekly reviews.
You are not just managing care — you are running a business.
What Success Looks Like
Within your first 6–12 months, you will:
- Establish Woven as a trusted provider in the community.
- Build a stable caregiver bench with strong retention.
- Develop a reliable referral pipeline.
- Achieve consistent client growth.
- Deliver high client satisfaction and strong documentation compliance.
- Reach financial breakeven per launch targets.
Who You Are
You are:
- Entrepreneurial and comfortable building from zero.
- Clinically competent and confident conducting assessments.
- Relationship-oriented and strong in community-facing environments.
- Highly organized and disciplined with systems.
- Calm under pressure and decisive in problem-solving.
- Motivated by accountability and measurable outcomes.
This role is ideal for someone who wants ownership, not just a job description.
Qualifications
- Bachelor’s degree preferred.
- RN, PT, or MSW license preferred
- 2 years supervisory experience in healthcare or service-based environments.
- Experience working with older adults.
- Strong communication and leadership skills.
- Financial and operational literacy.
- Reliable transportation and valid driver’s license.
- Preferred: Sales or marketing experience (i.e. med device or pharmaceutical sales)
Experience in home care, home health, senior services, or healthcare operations strongly preferred.
Compensation & Growth
- Competitive base salary.
- Performance-based bonus tied to growth, quality, and operational metrics.
- Significant leadership growth opportunity as Woven expands into additional markets.
- Ongoing mentorship and executive-level exposure.
This is a high-visibility leadership role within a rapidly growing organization.
