Woundology Clinical Coordinators (LPN) work comprehensively with providers to deliver wound care management for their patients. Woundology is seeking an experienced Licensed Practical Nurse to work 100% Remote – LPN Nurse (LPN) with at least 2 years of direct patient-facing work experience; that thrives in a fast-paced environment, is self-motivated, has impeccable attention to detail, and values the impact they can have on a patient’s healthcare journey. Our home care LPNs will use cutting edge technology and clinical knowledge to provide exceptional care to patients who require one-to-one care and attention in their homes.

- Fully remote position - Remotely in home and individual patient homes
- Consistent schedule - Full-Time Monday – Friday, no weekends
- Manage patient census with a resolution-driven approach to close gaps in clinical and non-clinical patient care.
- Identify and coordinate visits for their wound care interventions weekly
- Provide patient education
- Perform Ultamist Therapy for individual patients in their homes
- Assist in ensuring timely delivery of services to your patients
- Resolve patients' questions and create an open dialogue to understand needs and procedures
- Assist/Manage referrals and appointment scheduling
- Assist with sending documentation to home healthcare teams
- Active State License (LPN) (NLC) (LVN)
- Technical aptitude – Microsoft Office Suite and EMR systems
- Wound Care experience required
- Excellent written and verbal communication skill
- Must have active high-speed WI-FI
- Must have a HIPAA-compliant workspace
- Must have drivers license and own vehicle for travel to patients homes
- This position will be performing wound care and ultramist procedures in the home so being able to stand and sit or bend is a MUST
- This position will require the ability to speak clearly and listen attentively, often by telephone and in person
- The position will require the ability to understand, process, and document procedures each day after completion for Providers to cosign
