Licensed Practice Nurse (LPN)

Woundology Clinical Coordinators (LPN) work comprehensively with providers to deliver wound care management for their patients. Woundology is seeking an experienced Licensed Practical Nurse to work 100% Remote – LPN Nurse (LPN) with at least 2 years of direct patient-facing work experience; that thrives in a fast-paced environment, is self-motivated, has impeccable attention to detail, and values the impact they can have on a patient’s healthcare journey. Our home care LPNs will use cutting edge technology and clinical knowledge to provide exceptional care to patients who require one-to-one care and attention in their homes.

Requirements

• Manage patient census with a resolution-driven approach to close gaps in clinical and non-clinical patient care.
• Identify and coordinate visits for their wound care interventions weekly
• Provide patient education
• Perform Ultamist Therapy for individual patients in their homes
• Assist in ensuring timely delivery of services to your patients
• Resolve patients' questions and create an open dialogue to understand needs and procedures
• Assist/Manage referrals and appointment scheduling
• Assist with sending documentation to home healthcare teams
  
  
  

Additional Requirements:

• Active State License (LPN) (NLC) (LVN)
• Technical aptitude – Microsoft Office Suite and EMR systems
• Wound Care experience required
• Excellent written and verbal communication skill
• Must have active high-speed WI-FI
• Must have a HIPAA-compliant workspace
• Must have drivers license and own vehicle for travel to patients homes
  
  
  

Physical Requirements:

• This position will be performing wound care and ultramist procedures in the home so being able to stand and sit or bend is a MUST
• This position will require the ability to speak clearly and listen attentively, often by telephone and in person
• The position will require the ability to understand, process, and document procedures each day after completion for Providers to cosign
• Standing Frequently, Walking Frequently, Lifting Frequently up to 50 lbs, Carrying Frequently up to 50 lbs, Balancing Occasionally, Stooping Frequently, Kneeling Frequently
