Nurse Practitioner – Advanced Wound Care (Full-Time | Home-Based Care)
Based in Lake Villa, IL | Serving Lake, McHenry, Cook, DuPage, DeKalb, and Winnebago Counties
Salary: $125,000/year
4 or 5-day work week | 25–30 patients/week
Wound Wellness PLLC is a nurse practitioner-owned and operated wound care practice founded in February 2024, providing advanced wound care in the comfort of patients' homes and in-clinic. We are a mission-driven, rapidly growing startup seeking a full-time Nurse Practitioner who is compassionate, motivated, and ready to make a difference.

Position Highlights:
- Provide advanced wound care including:
- Sharp debridement
- Ultramist therapy
- Wound vac application
- Biopsies
- Tissue graft placement
- See 25–30 patients per week
- Flexible schedule – choose 4 or 5 working days
- Maintain weekly continuity of care
- Visits primarily conducted in patients’ residences
- Looking for someone to service the Chicago area

Ideal Candidate:
- Licensed Nurse Practitioner in the state of Illinois
- Live in or nearby the Chicago area
- New grads welcome!
- Home health or mobile wound care experience preferred
- Strong work ethic and adaptability
- Comfortable working independently
- Passionate about patient advocacy and continuity of care
- Excited to contribute to a growing and evolving practice
