Job description:
As a Wound Care Technician, you will play a vital role in ensuring the seamless delivery of healthcare services to our patients. Your attention to detail, excellent communication skills, and proactive approach will contribute to the overall quality of patient care. This position requires travel to cover the assigned territory. The responsibilities include:
- Appointment Coordination: Reach out to patients a day before their scheduled appointment to verify the appointment's suitability and absence of any scheduling conflicts. Ensure the appointment aligns with the patient's availability and needs.
- Communication Management: While en route to a patient's home, initiate a call to the patient or caregiver, keeping them informed about your estimated arrival time and addressing any immediate concerns.
- Appointment Scheduling: Organize weekly appointments with Wound Expert for providers, ensuring timely and effective patient consultations.
- Appointment Updates: In the event of any changes made to a provider's appointment day, promptly inform both the patient and the home health agency about the modifications.
- Supply Oversight: Confirm that providers have adequate supplies required for their appointments, facilitating smooth patient care.
- Data Documentation: Record wound measurements and capture photographs using the Wound Expert platform, ensuring accurate and comprehensive documentation for each patient's condition.
- Patient Information Accuracy: Verify that all patient information gathered during the initial visit, including insurance details, pharmacy contacts, medication lists, and allergies, are correct and up to date.
- Insurance Verification: Regularly confirm that patients' insurance information remains unchanged during every visit, preventing any discrepancies that could affect their healthcare coverage.
- Referral Assistance: Assist providers in sending necessary faxes and referrals as requested. This involves coordinating home health referrals, clinic referrals, and other relevant documents. Only involve the In-take Department for referrals requiring prior authorization due to diagnostic study needs.
- Wound Vac Support: For patients utilizing wound vacs, ensure accurate and timely wound measurements every two weeks. Transmit this information via fax to the designated Durable Medical Equipment (DME) company responsible for supplying the wound vac.
