Key Responsibilities:
- Provide direct wound care treatment under the supervision of a Nurse Practitioner (NP) or Physician.
- Perform wound assessments, dressing changes, debridement assistance, and infection control measures.
- Assist with advanced wound care techniques, such as negative pressure wound therapy and compression therapy.
- Educate patients, caregivers, and facility staff on wound prevention and treatment strategies.
- Document patient encounters in an EMR system in compliance with HIPAA and healthcare regulations.
- Monitor patient progress and report significant changes to the supervising provider.
- Coordinate with home health agencies, physicians, and other healthcare professionals to ensure continuity of care.
- Travel to patients’ homes, skilled nursing facilities, and long-term care settings within the assigned service area.
- Current LPN license in Nevada.
- Minimum of 1 year of experience in wound care, home health, long-term care, or a related field.
- Wound Care Certification (WCC) or similar is preferred but not required.
- Strong ability to work independently and autonomously in the field.
- Excellent communication and documentation skills.
- Must have a valid driver’s license and reliable transportation for travel within the service area.
