Nurse Practitioner

Job Overview
We are seeking a dedicated and skilled Nurse Practitioner to join our healthcare team. Wound care experience a plus! The ideal candidate will conduct comprehensive wound assessments, including pressure injuries, diabetic foot ulcers, venous and arterial ulcers, surgical wounds, and traumatic wounds.

Duties
As a Nurse Practitioner, you will be responsible for:

• Develop and implement individualized care plans in collaboration with patients, families, and interdisciplinary teams.
• Perform advanced wound care procedures such as debridement, dressing changes, negative pressure wound therapy, and infection management.
• Educate patients, caregivers and facility staff on wound care best practices and prevention strategies.
• Prescribe medications, treatments, and durable medical equipment as appropriate.
• Monitor wound healing progress and adjust treatment plans as needed.
• Maintain accurate, timely, and compliant electronic medical records.
• Coordinate with referring physicians, home health agencies, and other healthcare providers to ensure continuity of care.
• Travel within the designated service area, managing your schedule to optimize patient care and efficiency.

Experience
Candidates should possess the following qualifications:

• Active Nurse Practitioner license (state-specific, in good standing).
• Board certification as an NP (Family, Adult-Gerontology, or Acute Care preferred).
• Minimum 1-2 years of experience in wound care, dermatology, surgery, or related specialty (WOCN certification a plus).
• Current DEA registration (if prescriptive authority required).
• Valid driver's license and reliable transportation.
• Strong clinical assessment, critical thinking, and decision-making skills.
• Excellent communication and patient education abilities.
• Comfortable working independently in divers community settings.
