Key Responsibilities:- Administer medications and treatments as prescribed by physicians.
- Monitor and assess patients' health status, documenting changes appropriately.
- Assist residents with activities of daily living and ensure their comfort and safety.
- Collaborate with the interdisciplinary healthcare team to implement effective care plans.
- Maintain compliance with facility policies, state and federal regulations.
- Provide emotional support and education to patients and their families.
Requirements
- Current and valid NY State LPN license.
- Minimum one year of experience in a long-term care or similar healthcare setting.
- Strong communication, assessment, and organizational skills.
- Ability to work efficiently both independently and as part of a healthcare team.
- Current Basic Life Support (BLS) certification.
