Position Overview:
- Deliver professional nursing care to residents in long-term care settings
- Administer medications and treatments as prescribed by physicians
- Monitor and assess residents' health status and report any changes promptly
- Collaborate with healthcare team members to develop and implement individualized care plans
- Maintain accurate documentation of resident care and health information
- Ensure compliance with facility policies, procedures, and regulatory standards
- Current and valid NY State LPN license
- Minimum of 1 year experience in long-term care nursing preferred
- Basic Life Support (BLS) certification
- Strong clinical skills and attention to detail
- Effective communication and interpersonal skills
- Ability to work collaboratively within a multidisciplinary healthcare team
- Compassionate and patient-centered approach to care delivery
