Key Responsibilities:
- Administer medications and treatments as prescribed by physicians
- Monitor and assess patients' health status, documenting changes appropriately
- Assist residents with activities of daily living and ensure their comfort and safety
- Collaborate with the interdisciplinary healthcare team to implement effective care plans
- Maintain compliance with facility policies, state and federal regulations
- Provide emotional support and education to patients and their families
- Current and valid NY State LPN license
- Minimum one year of experience in a long-term care or similar healthcare setting
- Strong communication, assessment, and organizational skills
- Ability to work efficiently both independently and as part of a healthcare team
- Current Basic Life Support (BLS) certification
Salary : $32 - $35
