Job Overview

Duties

• Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor patient health by checking vital signs such as blood pressure, pulse, and temperature regularly
• Support medication administration following care plans and HIPAA regulations to ensure privacy and safety
• Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
• Conduct patient observation to identify changes in condition or behavior and report findings promptly
• Maintain a clean and safe environment through light housekeeping tasks such as cleaning, laundry, and organizing personal spaces
• Use specialized equipment like Hoyer lifts or resident's to assist with mobility and prevent injury during transfers
• Follow detailed care plans designed by healthcare professionals to deliver consistent, high-quality care
• Provide emotional support and companionship to residents or clients, fostering a positive environment
• Document all care activities accurately in compliance with healthcare standards such as HIPAA

Requirements

• Prior experience working in assisted living facilities, nursing homes, group homes, or home & community care settings preferred
• Knowledge of dementia care, Alzheimer's care, memory care, hospice care, and working with individuals with developmental disabilities or autism is highly desirable
• Certifications such as CPR, First Aid, and HIPAA compliance are required or strongly preferred
• Ability to lift heavy objects safely using equipment like Hoyer lifts when necessary
• Strong observational skills for patient monitoring and recognizing urgent health concerns
• Excellent communication skills for collaborating with healthcare teams, residents' families, and other caregivers
• Flexibility to work various shifts including days, evenings, weekends or holidays as needed
