What are the responsibilities and job description for the Community Case Manager position at Woods Ventures?
Job Summary
As a Case Manager, you will play a pivotal role in empowering individuals and families to achieve their goals and improve their overall well-being.
You will provide essential support in helping Our radiology practice call doctor’s offices in the community with the goal of helping us increase the number of patients per day.
Practice Goal: 12 patients per day, per modality for MRI, CT scan, and X-ray patients.
What you’ll do
- Call doctor offices in the local healthcare community to ask if they are seeking diagnostic imaging referrals for their patients
- Conduct comprehensive assessments to identify referral network needs and develop tailored service plans.
- Collaborate with doctor offices in the community to help them gain access to high quality diagnostic images efficiently
- Basic qualifications
- Strong understanding o healthcare private practice environment and patient flow.
Preferred qualifications
- Experience in social work, podiatry, ENT, orthopedic surgery, chiropractic, or related fields.
- Familiarity with Medicare processes.
- Proven ability to drive results
