The Impact You'll Make

As a Per Diem LCSW/LMSW, you will be a trusted advocate and emotional anchor for residents and their families. Your expertise will help navigate complex emotional, social, and care-related challenges, ensuring residents feel supported, understood, and empowered. Whether you’re guiding a family through a care transition, facilitating a support group, or helping a resident adjust to a new environment, your work will directly enhance quality of life. You will be part of a involved and dedicated interdisciplinary team with a common goal of resident and family extreme satisfaction.

What You'll Do

  • Conduct psychosocial assessments for new admissions and ongoing residents.
  • Provide individual counseling and emotional support to residents coping with illness, loss, or life changes.
  • Facilitate family meetings to address care planning, discharge needs, and emotional concerns.
  • Collaborate with nursing, rehabilitation, and medical teams to develop and implement individualized care plans.
  • Assist residents and families in accessing community resources, benefits, and support services.
  • Document all interventions and assessments in compliance with state and federal regulations.
  • Participate in care conferences and contribute to interdisciplinary discussions.

What You'll Bring

  • Active LCSW/LMSW license in the state of practice.
  • Experience in a skilled nursing facility, long-term care, or geriatric setting preferred.
  • Strong understanding of psychosocial issues affecting older adults and their families.
  • Excellent communication, mediation, and crisis intervention skills.
  • Ability to work independently while collaborating effectively with a multidisciplinary team.
  • Compassion, patience, and a deep respect for the dignity of every resident.

Qualifications:

Qualifications

1.    A master’s degree in social work or is a Licensed Certified Social Worker and has pertinent experience in a health care setting;
2.    A bachelor's degree in social work, or in a related field, and has regular access through a contract which meets the regulations; or
3.    Four years of social work experience in a nursing home in New York State prior to October 1, 1990, as a social work assistant or case aide, and has regular access through a contract which meets the regulations

4.    Proficiency with computers and standard office software
