We are seeking a compassionate, organized, and clinically skilled Clinical Intake Nurse to support admissions and transitions of care for two nursing facilities - Woodland Oaks in Ashland, KY and Oakmont Manor in Flatwoods, KY. This role is ideal for an LPN who excels in communication, clinical review, and coordinating seamless resident admissions.
Position Summary
The Clinical Intake Nurse is responsible for managing and coordinating the referral and admission process for two long-term care/skilled nursing facilities. The ideal candidate will work closely with hospitals, families, case managers, and facility leadership to ensure appropriate admissions and excellent continuity of care.
Responsibilities
- Review and evaluate incoming referrals for clinical appropriateness
- Coordinate admissions with hospitals, physicians, families, and interdisciplinary teams
- Verify insurance and obtain necessary authorizations
- Communicate effectively with referral sources and facility staff
- Ensure all admission documentation is accurate and complete
- Assist with bed management and census development
- Maintain knowledge of Medicare, Medicaid, and managed care requirements
- Support smooth resident transitions into the facilities
Qualifications
- Active LPN license in good standing
- Experience in skilled nursing, long-term care, case management, or admissions preferred
- Strong clinical assessment and communication skills
- Knowledge of insurance authorization processes and discharge planning
- Ability to multitask and work independently between two facilities
- Proficient computer and documentation skills
