Woodland Manor Nursing and Rehab is seeking a compassionate and energetic Activities Assistant – Memory Care to join our skilled nursing facility in Elkhart, IN. This role works under the direction of the Director of Activities to provide engaging, meaningful programs specifically designed to support residents in our Memory Care community.
Position SummaryThe Activities Assistant – Memory Care helps plan, organize, and conduct individualized and group activities that enhance the emotional, social, cognitive, and recreational well-being of residents with memory impairment. This role focuses on promoting dignity, independence, and quality of life through structured and therapeutic engagement.
Key ResponsibilitiesProvide memory care–appropriate activities that support cognitive function, routine, and emotional comfort
Ensure residents’ emotional, recreational, and social needs are met on an individual basis
Assist residents with daily activities such as writing letters, making phone calls, and participating in structured routines
Escort residents to scheduled activities and assist with safe participation
Maintain accurate documentation of resident participation and engagement
Assist with mail delivery, plant and flower care, pet/fish care, and preparation of the monthly newsletter
Support fundraising efforts and make announcements over the PA system
Conduct individual and group activities that promote stimulation, relaxation, communication, and creative expression
Provide and supervise the use of craft supplies, materials, and equipment
Encourage residents to participate in crafts, painting, music, games, and other therapeutic activities designed for memory care residents
High school diploma or GED preferred
Previous experience working in memory care, long-term care, or with elderly or cognitively impaired individuals is preferred
Must be 18 years of age or older
