POSITION TITLE: Licensed Practical Nurse, PRN-LPN Assisted Living
REPORTS TO: RN or Administrator
POSITION SUMMARY:
Woodland Hills Assisted Living is seeking a dependable and clinically strong PRN LPN to support our nursing team. This role provides high-quality, resident-centered care including medication administration, monitoring resident conditions, documentation, and supporting caregivers during assigned shifts.

This is a lower-acuity setting compared to hospital or skilled nursing, allowing you to focus on quality care in a more controlled, less stressful environment.
COMPENSATION AND SCHEDULE:
- Pay: $22.00 – $25.00 per hour (based on experience)
- Performance-based bonuses including facility census incentives
- Flexible PRN scheduling – pick up shifts based on your availability
- Preferred availability: minimum of 2–4 shifts per month
- Consistent attendance required to remain active in PRN pool
KEY RESPONSIBILITIES:
- Administer medications and treatments per physician orders
- Monitor residents and report changes in condition
- Complete accurate documentation and incident reports
- Follow up on orders, labs, and provider instructions
- Assist with admissions and service plan implementation
- Supervise and support caregiver staff
- Maintain compliance with WV assisted living regulations
- Follow infection control and safety protocols
- Communicate effectively with residents, families, and team
QUALIFICATIONS:
- Active LPN license in West Virginia required
- Graduate of an accredited nursing program
- Experience in assisted living or long-term care preferred
- Ability to work independently and make sound clinical decisions
