Wood River Health has an exciting opportunity to join the team as Health Equity Zone (HEZ) Program Director. Wood River Health is the backbone agency for the Health Equity Zone (HEZ) in Washington County called Healthy Bodies Healthy Minds.  This role requires regular on-site presence; therefore, candidates must be local to the Washington County/Rhode Island area.

The HEZ Program Director is a grant-funded position, that will coordinate and provide oversight of the Health Equity Zone projects in the Washington County communities. This position facilitates hyper-local, community driven, collective impact initiatives that address barriers to health with the community. This position develops high functioning partnerships with teammates, residents, and organizations to a) identify and address community needs and priorities, b) build and manage measurable and attainable multi-year work plans to improve health outcomes and equity c) develop and manage shared goals to foster collective impact d) manage budgets and subcontracts e) ensure grant deliverables and reporting requirements are met on time. In addition, this position requires exceptional communication, organization, and project management skills.

HEZ Program Director essential duties include, but are not limited to:

  • Responsible for utilizing community engagement strategies, resources, and tools to support the implementation of assigned deliverables within targeted program areas.
  • Responsible for overseeing the administrative areas of programming, ensuring efficient program and office operations by providing a range of support both within and outside of the office, and supporting the capacity building of community partners.  
  • Supporting the development of Community Engagement programming to include building the organizational capacity of partners and neighborhood coalitions, establishing programmatic connections to effectively implement place-based initiatives, representing the program at related working committees, and helping to promote and monitor the implementation of the strategic plan.  
  • Communicate effectively among partners.
  • Collect data and complete reports as required.
  • Build and maintain relationships with community partners to support effective programming in all areas of responsibility.
  • Manage contracts, grant budgets, and deliverables for the HEZ and subcontracts through the HEZ.
  • Support community trainings, outreach, and marketing activities related to the HEZ priorities.
  • Conduct or oversee community needs assessments.
  • Develop action plans associated with the results of the needs assessments.
  • Participate in RI Department of Health required HEZ trainings.
  • Supervise interns as appropriate.
  • Maintain strict confidentiality in all matters.
  • Operate within the scope of the Health Information Portability and Accountability Act to safeguard the privacy of protected patient health information. 
  • Enliven and support the mission, vision, and values of Wood River Health.
  • Adhere to organizational policies and procedures and Wood River Health Compliance Program Standards.
  • Performing other duties as assigned to meet business needs.

EDUCATION AND EXPERIENCE: 

  • Bachelors in related field (social work, community health, public health, health education, etc.) required; masters preferred. 
  • Must have experience working with community-based partners and resident leaders to develop and implement projects, promotion campaigns and/or programs. 
  • Experience in program/project management and performance measurement. 
  • Experience supporting community-led efforts to address inequities and advance social change. 
  • Interest and/or experience in policy work.
  • Ability to work independently and effectively in a team environment, collaborate with partners.
  • Highly organized and strong interpersonal skills. 
  • Must have flexibility to work occasional evenings and/or weekends.
  • Ability to effectively use MS Office Suite, Internet, and email.
  • Cultural sensitivity necessary to work with a diverse patient and staff population.

Salary : $72,000 - $82,000
