PURPOSE OF POSITION
The purpose of this position is to provide case management and coordination of the admission process for residents and resident families.
ESSENTIAL FUNCTIONS
In addition to consistent and reliable attendance, the following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.
Serves as case manager for managed care/skilled residents; ensures proper equipment is ordered; monitors resident’s progress to ensure proper ADL support measures are in place; tracks acute hospital admissions to ensure re-admission of patients; assists with resident transition from short-term stay to long-term care as needed; fields resident and/or family complaints and forwards issues to appropriate department.
Coordinates the pre-admission process; verifies payor source and insurance coverage; oversees the pre-certification and re-certification process; ensures level of care is completed prior to admission; conducts pre-admission review with Director of Nursing; schedules new resident’s arrival at facility; ensures resident rooms are properly equipped with necessary amenities prior to arrival; fields questions from family members.
Conducts admission paperwork and process; completes nursing packet and notifies staff of new admission; assigns patient numbers; inputs patient information into computer including MDS program and on resident chart; creates name tags for charts, doors, and beds.
Conducts facility tours; assists with marketing to hospitals, physicians, community organizations, and social service organizations.
Serves as the facility’s HIPPA compliance officer.
Collects appropriate data, prepares and submits reports as assigned; prepares letters, correspondence, and reports; files and maintains departmental records.
Attends required meetings, consultations, in-services, etc; Participates in industry trade shows and business development activities as assigned.
ADDITIONAL FUNCTIONS
Answers phones and performs various clerical duties as needed.
MINIMUM QUALIFICATIONS
Registered Nurse, Licensed Practical Nurse, or Bachelor’s degree in social work, healthcare administration or related field; supplemented by one years’ experience in the field or any equivalent combination of education, and experience that provides the requisite knowledge, skills, and abilities for this job. Knowledge of managed care, Medicaid, and Medicare preferred. Valid driver’s license preferred.

Pay: $26.89 - $31.19 per hour
