Infusion Nurse

Description:Wondercare, Inc., a Partner of Soleo Health, is seeking a Per Diem Home Infusion Nurse to support our Hawthorne, NY patients.The Position:The Infusion Nurse provides primary patient care for specifically assigned patients to include assessments, care planning, evaluation and education of patients receiving infusion therapy and/or other nursing services in accordance with the care matrices and best practices in the home or alternate site settings. Responsibilities Include:Provide the majority, if not all of the direct care for all specifically assigned patients including initiation and maintenance of all communications with appropriate care team members including pharmacists, physicians, dieticians, home patient representatives and any appropriate community resource staff members.Evaluate and educate patient/care partner’s ability to safely administer the home infusion therapy and provide additional instruction as indicated.Evaluate the home environment where the care is to be provided and identify interventions that may be required to render the home care environment safe and appropriate for the prescribed care.Develop, in conjunction with other clinical team members, each patient’s Care Plan and evaluate patient’s response to therapy during each home visit or encounter.Appropriately discharge primary patients and complete discharge summary within 7 days of discharge.Develop and maintain caseload scheduling and communications on a weekly and monthly basis in updating schedule with changes.Document patient visit to physician or lab drawing station on calendar.Coordinate care via live telephone conversations and/or voicemail with the team as determined by the urgency of information to be relayed.Document all communications at the time of service.Rotational after hour and weekend on call, and as neededComputerized documentation of patient visits accurately and in a timely mannerSchedule:Must be able to work Per Diem up to 10 hours per week, variable scheduleMust be knowledgeable of IV access devices to include: IV Lines, ports, and administration of infusion medicationsMust be able to provide infusions to patients in their homesMust be able to drive up to 60 miles to see patients as neededMust have properly registered/insured vehicle to see patientsMust be licensed as an RN in the state of New YorkRequirements:Minimum 3 years nursing experience with background infusion therapyRN degree- BSN preferredCurrent nursing license for applicable stateExperience in ICU, or Cardiac Care preferredExcellent customer service skillsProficient in Microsoft Excel, Word, and OutlookKnowledgeable and competent in care and maintenance of vascular access devicesCompetent in peripheral IV placement skillsCurrent CPR CertificationPICC/Midline placement a plusCRNI a plusMust own properly registered and insured vehicle to use for patient visitsValid state driver’s licenseAbility to multi-task and have critical thinking skillsRoad warriorWondercare is committed to diversity, equity, and inclusion.
