Position Description: CARE COORDINATOR
Care Coordinator’s provide clinical services to residents and families assigned to caseload. Care Coordinator’s serve as a member of the interdisciplinary team supporting Wise Care Services’ Philosophy of serving residents and their families.
Essential Responsibilities:
- Provide individual, family, and group interventions/meetings per standard for residents on caseload.
- Complete appropriate documentation within time frame defined by Standards and Wise Care Services.
- Responsible for coordination of residents’ treatment from admission through discharge.
- Facilitate Monthly Treatment Plan Reviews / Weekly Progress Reports to ensure residents’ progress and discharge planning is occurring.
- Available to provide on-call support as determined by staffing needs.
- ***Must be able to successfully complete certification for Non-Abusive Psychological and Physical Intervention training, CPR/First Aid and other new hire training.
- The CARE COORDINATOR also acts as consulting supervisor to Direct Care Staff.
Essential Skills and Experience:
- Minimum Requirement: Bachelor’s degree in Psychology, Social Work, or other related field. Master’s degree in psychology, Social Work, or other human service degree, licensed in S.C. or that meets eligibility to acquire South Carolina licensure is preferred.
- Ability and desire to function successfully as part of a team.
- The ability to establish effective work relationships.
- Excellent verbal and written communication skills.
- Ability to be flexible and establish/meet deadlines.
- Additional duties as needed or assigned.
Additional:
Knowing the Resident
Staff, in conjunction with the Care Coordinator, are expected to have regular discussions and get to know the child more thoroughly (their interests, likes, dislikes, goals, fears), to make sure that the resident’s individual needs are being addressed by the team, and to provide additional support as needed. In addition to the care team, the Care Coordinator liaises with other staff, the team and the Director regarding the needs of the child. In conjunction with direct care staff, they are also responsible for:
- ensuring that the child’s material needs (including clothing, toiletries and personal items) are met;
- liaising with the staff to organize practical matters (for example, access to funds, schedules appointments, etc.);
- ensuring that special events relevant to the child are planned (such as the child’s birthday, family members’ birthdays, cultural events);
-assisting the child to maintain personal records (certificates, photographs, memberships); and
-participating in discussions, planning and reviews involving the child.
As a team, the Care Coordinator and Direct Care Supervisor also work together with the Director to ensure the following:
Essential Responsibilities:
- Assists with documenting grievances, terminations, absences, performance reports, and compensation information; also often involved in recruiting, hiring, and training new employees.
- Obtains applicant information; assists to screens applicants, coordinates admissions process; QA reviews.
Applicant must successfully complete background checks, educational and training requirements.

Pay: $43,780.46 - $50,000.00 per year
