JOB PURPOSE: Responsible for the coordination of all systems/services required for a multidisciplinary team approach to care. Assure quality and cost efficient care for the sub-acute, medically complex and rehabilitation patients. The Case Manager functions as a contact person for patient, family, health care team members and payer source.
POSITION REQUIREMENTS:
- Three years clinical experience with at least one in long term care required.
- Advanced communication, problem solving and interpersonal skills.
- Knowledge of Medicare and Medicaid regulations in long term care required.
