Health Services Coordinator (LPN)
Memory Care Community
Seeking a licensed LPN to support the RN/Health Services Director with resident care coordination, medication oversight, documentation, and communication with families and providers. Must hold a current, Washington State LPN license. Experience in assisted living or memory care preferred. Strong communication, organization, and teamwork skills required.
EDUCATION & EXPERIENCE
- Licensed Practical Nurse (LPN) or Medical Assistant preferred, or equivalent healthcare experience
- Minimum of one (1) year experience in assisted living, memory care, or healthcare setting preferred
- Experience supporting nursing or clinical teams preferred
- Continuing education in healthcare or care coordination encouraged
JOB CONDITIONS
- May require flexible scheduling, including evenings, weekends, and holidays
- May require coverage during emergencies or staffing shortages
- Occasional travel for training or meetings may be required
PHYSICAL DEMANDS
- Ability to walk, stand, bend, stoop, and sit for extended periods
- Ability to respond quickly in emergency situations
- Ability to lift, carry, push, or pull up to 50 pounds
- Ability to work at a computer for sustained periods
- Ability to communicate effectively via phone, radio, or in person
