In this role, you will provide comprehensive case management services, including conducting diagnostic evaluations, developing individualized treatment plans with measurable goals, offering crisis intervention when needed, and maintaining ongoing follow-up to ensure clients receive coordinated, high-quality care.
Key Responsibilities:
- Conduct mental health assessments and diagnostic evaluations
- Develop and implement individualized treatment plans
- Provide crisis intervention and ongoing client support
- Coordinate services and resources to meet client needs
- Monitor progress and adjust care plans as appropriate
Qualifications:
- Bachelor’s degree in Social Work, Psychology, or a related field OR
- A minimum of two years of experience in a direct service role (such as after-school programs, nursing homes, mental health settings, or client advocacy)
- 1–3 years of experience working with individuals receiving mental health services
