What are the responsibilities and job description for the Fitness Monitor Lead (Arapahoe) - Diabetes position at Wind River Family and Community Health Care?
Responsible for opening, closing and securing the facilities. Responsible for the appropriate use and cleanliness of all equipment. Required to provide a welcoming, inclusive, safe and enjoyable service oriented environment for participants at all times.
Work all scheduled shifts, with strict attention to being on time. Oversee the facility at all times to ensure the safety of the participants and security of the equipment. Must obtain certifications in fitness training, First Aid and CPR. Must maintain Personal Trainer Certification. Must have extensive knowledge of physical fitness equipment and the knowledge skills and ability to design physical fitness programs. Familiarize participants with safe and effective use of fitness, aerobic and free weight equipment. Provide service and assistance to all Fitness Monitors. Maintain equipment and report any safety or maintenance needs in written form to Diabetes Program Director. Clean and "wipe down" all equipment on a regular basis as scheduled with documentation. Following opening and closing procedures with documentation. Prepare incident and injury reports when appropriate. On Call as possible substitute and/or support. Respond to and follow all emergency procedures. Attend all mandatory staff meetings and workshops. Provide a friendly, upbeat and quality customer service environment at all times, for all participants. Ensure a safe environment for all participants. Practice safety and risk management. Coordinate physical activities for the community. Assists in participants education and personal fitness in a variety of settings (i.e. home, clinical, school, community.) Consults with the Diabetes Program Director about questions or problems related to the care of participants. Assists in the development of culturally sensitive participants' education materials. Assists with establishing policies and procedures. Submit required reports and records to the Diabetes Program Director.
Participates in the project quality improvement and evaluation program. Other duties as assigned to maintain and enhance the operation of the department.
