What are the responsibilities and job description for the Home Health Registered Nurse PRN position at WILSON COUNTY MEMORIAL HOSPITAL DISTRICT?
Summary: The RN provides skilled nursing care in the home adhering to State and Federal Regulations and based on Physician’s orders, makes initial visits, develops, implements, evaluates, and re-assesses patient care plans that consider the patient’s physiological, psychological, and safety needs following agency and nursing Standards of Care.
Essential Duties and Responsibilities: include the following. Other duties may be assigned.
- Responsible for collecting all OASIS data, developing the Plan Of Care, assuring that the POC is being followed, and planning for appropriate, timely discharge of patients from Home Health Services.
- Assigns skilled and home health aide patients to RNs, LVNs, and HH Aides in an appropriate and timely manner, giving consideration to the level of competence required by the patient.
- Coordinate patient care and treatment with physicians and care providers from other disciplines, and services (Physical Therapy, Occupational Therapy, Speech Therapy, HH Aide, etc.).
- Communicate information related to patient progress, needs, and/or condition changes to physicians, agency staff, patient, family and significant others as applicable.
- Demonstrates substantial knowledge of medications and their uses and knowledge related to the care of patients of all ages, especially geriatrics.
- Completes and submits OASIS and all other documentation in a timely manner.
- Supervises agency LVNs monthly and HH Aides every 14 days; develops HH Aides care plans.
- Responsible for assuring patients are scheduled appropriately and visits have been performed per ordered frequency.
- Participates in the “On-Call” rotation as directed by the Administrator.
- Achieves all goals and objectives in an ethical, moral manner, and in strict accordance with all Federal, State, and Local government statutes and regulations.
- Maintains confidentiality of patient information and respect for patient behavioral response to illness.
- Works as scheduled and follows attendance policies such as reports to duty on time, reports illness in advance of shift, accepts assignments and completes them using good time management skills, and make every effort to be on duty as scheduled.
Qualifications:
Education and/or Experience
Licensed Registered Nurse in the State of Texas; Graduate of an accredited school of nursing; CPR.
Other Skills and Abilities
- IV therapy, Wound Vac Therapy, and Venipuncture experience preferred
- Computer literacy and adaptability required
- Excellent organizational, communication, and judgment skills are a must.
- Maintain own internet accessible device with reliable internet access
- Maintain own cell phone with reliable network coverage
- Interpersonal skills necessary to communicate effectively with various levels of hospital staff.
Special Requirements: Reliable transportation with insurance coverage; TX Driver’s License & Current Car Insurance must be kept on file at all times.
