Responsibilities
- Conduct comprehensive assessments of patients’ physical conditions, including mobility, strength, and functional abilities.
- Develop and modify individualized care plans based on patient needs, clinical standards, and treatment goals.
- Deliver evidence-based physiotherapy interventions such as therapeutic exercises, manual therapy, trigger point therapy, and modalities tailored to each patient’s condition.
- Document patient progress accurately in electronic health records (EHR) systems following clinical documentation standards.
- Collaborate with healthcare professionals—including physicians, nurses, occupational therapists, and social workers—to coordinate care plans and discharge strategies.
- Educate patients and their families on treatment techniques, home exercises, and community resources to support ongoing recovery.
- Manage discharge planning processes by assessing patient needs for continued therapy or community support services.
Skills
- Strong knowledge of physiology, anatomy, orthopedics, neurology, geriatrics, pediatrics, and related medical terminology.
- Proven experience practicing in various physical therapy roles across inpatient hospital settings, outpatient clinics, home health environments, or rehabilitation centers.
- Familiarity with skilled nursing facilities.
- Ability to develop effective treatment plans that align with clinical standards and patient goals.
- Proficiency in electronic health records (EHR) management and EMR systems for accurate documentation of patient care activities.
- Excellent client communication skills to engage geriatric and pediatric populations—and foster a positive therapeutic environment.
- Experience with discharge planning processes and working within skilled nursing facilities to ensure smooth transitions from hospital to home or community care.
- Knowledge of medical terminology relevant to physiotherapy practices in skilled nursing facilities.
- Ability to work collaboratively with healthcare teams to optimize patient rehabilitation outcomes.
