Duties

• Conduct comprehensive assessments of patients’ physical conditions, including mobility, strength, and functional abilities, utilizing your physiology and anatomy knowledge.
• Develop and modify individualized care plans that align with patient goals and medical directives, ensuring optimal recovery outcomes.
• Deliver hands-on physiotherapy interventions such as trigger point therapy, orthopedics-focused treatments, and geriatrics-specific exercises to promote healing and mobility.
• Assist in discharge planning by coordinating with healthcare teams and providing education on home & community care strategies to support ongoing recovery.
• Document patient progress accurately within electronic medical record (EMR) systems, maintaining detailed records of treatments provided and patient responses.
• Collaborate with multidisciplinary teams.
• Educate patients and their families about treatment procedures, injury prevention techniques, and physiotherapy exercises to empower self-care at home.

Qualifications

• Valid licensure or certification as a licensed Physical Therapist (PT), with current registration where applicable.
• Proven experience in acute care hospital environments, outpatient clinics, or home health settings demonstrating adaptability across diverse clinical scenarios.
• Strong knowledge of medical terminology, physiology, anatomy, and orthopedic rehabilitation techniques.
• Experience in discharge planning, care plan development, and implementing therapy interventions such as trigger point therapy for musculoskeletal conditions.
• Familiarity with EMR systems for documentation and patient record management.
• Background in pediatrics, geriatrics is highly desirable.
