Position Summary:
The Social Services Director is responsible for planning, developing, and directing the overall operation of the Social Services Department in accordance with current federal, state, and local standards, guidelines, and regulations. This position ensures that the medically-related emotional and social needs of the residents are met and maintained on an individual basis.
Key Responsibilities:
Develop and implement social service programs that meet residents’ psychosocial needs
Conduct assessments and create individualized care plans
Advocate for residents' rights and promote independence and dignity
Participate in interdisciplinary care planning meetings
Serve as a liaison between residents, families, and staff
Provide counseling to residents and families as needed
Assist with admission, discharge planning, and transitions of care
Maintain documentation in accordance with regulatory requirements
Qualifications:
Bachelor's degree in Social Work or related field (Master’s preferred)
Minimum of 2 years’ experience in long-term care or healthcare setting
Knowledge of federal and state regulations pertaining to long-term care
Strong communication, organizational, and problem-solving skills
Empathetic, patient, and resident-focused approach
