Role Description
This is a part-time, on-site role for a Hospice Social Worker based around Oak Brook IL and surrounding cities. The Hospice Social Worker will provide emotional, social, and resource-based support to patients and their families during end-of-life care. Responsibilities include conducting psychosocial assessments, facilitating individualized care plans, coordinating discharge planning, and providing grief counseling. The role involves working closely with an interdisciplinary team to ensure patients receive holistic, compassionate care tailored to their needs.
Qualifications
- Experience in Medical Social Work and proficiency in Social Work principles
- Knowledge and expertise in End-of-Life Care and Grief Counseling
- Strong skills in Discharge Planning and coordinating resources for patients and families
- Master’s degree in Social Work (MSW) from an accredited institution
- Current licensure as a Licensed Clinical Social Worker (LCSW) or equivalent in Illinois
- Excellent communication, organizational, and interpersonal skills
- Experience in hospice or palliative care is highly desirable
