Full Job Description
We are currently looking for a part time (20 hours a week) LISW or LPCC. This is an in-person position. Duties include:
- Therapy
- Treatment Planning
- Progress Notes Review
- Assessments
- Transition/Discharge Summaries
Thorough documentation of screening/assessments, evaluations, notes, contacts made must be accurate and timely.
Position is responsible for coordinating care with the Case Manager and other care providers.
Must be a Licensed Professional Clinical Counselor or Licensed Independent Social Worker in Ohio.
