Our Social Services department plays an integral part of the multidisciplinary team by working to meet the psychosocial needs of our residents and supporting their overall quality of life throughout the various levels of care.

The Social Worker conducts assessments, provides family and resident education, completes audits for ongoing quality improvement and facilitates the transition of residents between levels of care. On a regular basis, the Social Worker along with the Director of Social Services provides education and insight on Resident Rights.
Essential responsibilities include but are not limited to:
- To identify and support in the social and emotional needs of the resident
- To work and collaborate with other members of the multidisciplinary team to implement a coordinated approach to providing the highest quality care
- To perform the advocacy role and represent the resident to the multidisciplinary team, and to the family, as appropriate
- To coordinate discharge services for residents, i.e. personal care in-house services; meals; transportation; respite care; adult day care; support groups
- Participate in care planning and care team meetings and is an integral member of the interdisciplinary team responsible for the overall well-being of our residents
- Licensed Social Worker in the State of New Jersey
- Three years of experience in a multi-level long-term care setting
- Subacute experience a plus
- Working knowledge of State and Federal requirements related to long-term care
- Effective and professional communicator to an older population and their families
