Education/Experience
- High school diploma or GED. One-year experience as an LPN. Experience in ID/DD preferred.
- Word processing, database, Internet software, E-mail.
- NYS License as a Practical Nurse.
- Certification in First Aid and CPR.

Essential Duties and Responsibilities include the following. Other duties may be assigned.
- Acts as the primary medical coordinator for individuals in the residence; Interviews individuals to determine medical problems and conditions.
- Acts as primary staff person for documentation of medication administration in the medication database.
- Reviews medication administration records on a regular basis.
- Administers specified medication and notes time and amount on medication administration database.
- Assists Registered Nurse with physical examinations as needed.
- Reviews physician's orders and follow-up needs with Registered Nurse.
- Administers First Aid and CPR on an emergency and as needed basis.
- Observes individuals and reports adverse reactions to medication or treatment to the program RN.
- Counsels individuals on good health habits.
Additional Requirements:
- A valid NYS driver's license and ability to drive and driving record acceptable for agency insurance coverage are required.
- Medication Administration certification (or eligibility for certification).
- SCIP-R certification (or eligibility for certification) based on program need.
Salary : $24 - $33
