Summary: As a community Habilitation DSP you are matched to an individual based on likes, location and availability. Generally you will be going out into the community with the individual. This position requires a great amount of commitment, self-motivation and solid communication skills.

Requirements
- High school diploma or general education degree (GED).
- A valid NYS driver's license and ability to drive and driving record acceptable for agency insurance coverage are required.
- Access to a vehicle needed to provide services.

The work is usually one to one
- Participates in a person centered approach to program plan development and participates in activities to build competencies and integrate individuals into their communities.
- Assists individuals we serve to identify social, recreation and other community resources the individual may wish to access and devise plans to achieve that access.
- Negotiates schedules and daily activities directly with the individual and/or caregiver. Establish and maintain regular hours with individual(s) that you work with
- Completes all mandated reporting requirements by Wildwood and regulating agencies.
Additional Requirements:
- A valid NYS driver's license and ability to drive and driving record acceptable for agency insurance coverage are required.
- May need daily access to a vehicle based on program needs.
Salary : $18
