Through our Whole Health Wellness Institute, we focus on optimizing patients before and after surgery by integrating lifestyle guidance, education, and ongoing support. As part of our commitment to whole-body wellness, surgical patients participate in our Remote Physiologic Monitoring (RPM) and Remote Therapeutic Monitoring (RTM) programs.
We are seeking a compassionate, organized, and tech-savvy **Wellness Navigator** to join our growing team.
Position Summary
The Wellness Navigator plays a crucial role in supporting patients throughout their surgical journey. This role focuses heavily on daily patient communication—primarily by phone and virtual visits—to help patients navigate our RPM and RTM programs, understand their care plans, and maintain engagement before and after surgery.
Key Responsibilities
· Serve as the primary point of contact for patients enrolled in our RPM and RTM programs.
· Conduct frequent telephone and virtual check-ins, making up the majority of daily tasks.
· Support all hip and knee surgical patients beginning prior to surgery and continuing through their postoperative recovery period.
· Review patient-reported data, activity tracker information, symptoms, and trends, escalating concerns to the clinical team as needed.
· Educate patients on the purpose, expectations, and steps involved in their monitoring programs.
· Assist patients with device setup, troubleshooting, and proper use of digital monitoring tools.
· Maintain accurate documentation of all interactions in the electronic medical record.
· Collaborate with orthopedic surgeons, APPs, wellness clinicians, and administrative staff to ensure seamless patient care.
· Reinforce wellness principles, lifestyle recommendations, and recovery milestones to optimize patient outcomes.
Qualifications
· Preferred clinical backgrounds include: Athletic Trainer (AT), Licensed Practical Nurse (LPN), Registered Nurse (RN), or candidates with similar healthcare degrees/certifications.
· Prior healthcare or patient-support experience strongly preferred.
· Strong communication skills with a calm, empathetic approach.
· Comfortable spending most of the day on the phone or in virtual patient visits.
· Experience with remote monitoring programs (RPM/RTM) is a plus but not required—training provided.
· Proficiency with digital devices, patient portals, and EMR systems.
· Ability to multitask, prioritize, and manage a large patient panel.
· Passion for patient education, wellness, and proactive care.
