Case Manager - LVN

Job Overview
We are seeking a dedicated and compassionate Case Manager - LVN (Licensed Vocational Nurse) to join our dynamic healthcare team. Responsibilities

• Conduct comprehensive patient assessments, including vital signs and physiological evaluations, to determine care needs across inpatient and outpatient settings.
• Develop and implement individualized care plans in collaboration with physicians, specialists, and other healthcare professionals.
• Coordinate discharge planning by arranging post-acute care services, outpatient follow-ups, and ensuring proper documentation in medical records.
• Manage utilization review processes by evaluating diagnostic evaluations and supporting appropriate resource allocation within managed care frameworks.
• Facilitate communication with patients and families to provide education on treatment plans, medication management, and health maintenance strategies.
• Maintain compliance with HIPAA regulations and ensure confidentiality of all medical records and patient information.
• Participate in case management activities related to ICU, trauma centers, emergency medicine, geriatrics, behavioral health, pediatrics, and other specialized units requiring acute or post-acute care expertise.

Requirements

• Valid LVN license with current state certification required; additional certifications in critical care or trauma nursing preferred.
• Proven experience in hospital medicine, emergency room settings, or intensive care units such as ICU or PICU (Pediatric Intensive Care Unit).
• Strong knowledge of medical terminology, physiology, and diagnostic evaluation techniques essential for accurate patient assessment.
• Experience working within Level I or Level II trauma centers or similar high-acuity environments is highly desirable.
• Familiarity with hospital experience including medical-surgical units, post-acute care facilities, outpatient clinics, and managed care operations.
• Excellent communication skills to effectively coordinate multidisciplinary teams and engage with patients from diverse backgrounds.
• Ability to handle sensitive information with discretion while adhering to HIPAA guidelines.
• Strong organizational skills for managing medical records, discharge planning documentation, and intake processes across inpatient and outpatient settings.
