Position Summary[LZ1] 

As a Structural Heart APP, the PA/NP will work in an administrative and clinical capacity with collaborating physicians, clinical staff, patients and their families, insurance companies, and vendors, in the planning, evaluation, and coordination of care for our Structural Heart patients across the entire continuum of care, including but not limited to, preprocedural / post-procedural care in the outpatient and inpatient clinical settings.

Essential Functions and Responsibilities Includes the Following:

  1. Understands and adheres to the WPH Performance Standards, Policies and Behaviors.
  2. Ensure effective communication with all parties, delivering superior patient care and experience.
  3. Coordinates overall clinical program and ensures processes are in place to ensure smooth operation and patient flow for Structural Heart patients referred to the Valve Clinic.
  4. Coordinates the clinical interdisciplinary team and services for pre-procedural evaluation prior to heart valve surgery; including but not limited to Transcatheter Aortic Valve Replacement (TAVR).
  5. Participate in multidisciplinary meetings to present professional-based presentations, meetings, and communication skills.
  6. This position may be involved in research clinical trials, patient and professional educational programs, and patient follow-up.
  7. The role of the Structural Heart PA/NP will evolve over time to meet the needs of the patients and of the Structural Heart Program.
  8. Excellent written and verbal communication skills
  9. Gathers all patient data points needed for the ACC NCDR TVT and LAAO registries, in accordance with all definitions and prior to submission deadlines, ensuring follow visits occur within the specified windows.
  10. Knowledge of insurance coverage systems, including but not limited to co-payments, prior authorizations, and referrals is preferred.
  11. Coordinates safe discharge/transfer of Structural Heart patients.
  12. Provides education to patients and/or families about preventive care, medical issues, and use of prescribed medical-surgical

treatments and/or medications pertinent to the Structural Heart patient's condition.

  1. Writes orders for medications, laboratory work, and diagnostic tests. Interprets laboratory and diagnostic test results
  2. Documents all medical evaluations, diagnoses, procedures, treatments, outcomes, education, referrals, and consultations

consistent with all regulatory standards and evidenced-based standards of care. Maintains documentation compliance for

appropriate coding and billing

  1. Able to evaluate medical/surgical problems of Cardiac Services patients and appropriately requests consults to other

specialties when indicated

  1. Performs all other related duties as assigned.

Education & Experience Requirements

  • Graduation from Accredited Physician Assistant or Nurse Practitioner Program
  • Current National Certification.
  • Minimum of 2 years Cardiothoracic Surgery, Invasive Cardiology or Cardiology APP experience.  Previous Transcatheter Aortic Valve Replacement experience highly desired
  • Familiarity with EPIC EMR, MacLab, and clinical data collection highly desired
  • Experience with clinical trial management, quality and data abstraction or clinical research coordination experience a plus.
  • Current New York State Licensure
  • Current DEA Licensure
  • Current AHA  BLS and ACLS Certification
  • Current Infection Control Certification

Physical/Mental Demands/Requirements & Work Environment

  • May be exposed to chemicals necessary to perform required tasks. Any hazardous chemicals the employee may be exposed to are listed in the hospital’s SDS (Safety Data Sheet) database and may be accessed through the hospital’s Intranet site (Employee Tools/SDS Access). A copy of the SDS database can also be found at the hospital switchboard, saved on a disc.
